23 yr, G2P1 (vaginal delivery 01-03-01)
Last menses 09-20-01
First visit 12-18-01 (12.5 wk amenorrhoea)
Scan made with a HITACHI EUB-405, vaginal probe not available at that moment.

(fig 3)  I also thought it was a bicornus uterus or didelfus, or a septum uterus. And so...the second visit, 18 wk by u.s., and offered this pic (it´s a bad pic)

It may be a left interstitial implantation in a bicornuate uterus. The right "sac" could be a cast.

I agree with Dr. Lyons, this may be a case of bicornuate uterus... the decidua basalis implantation appears it may be over the birth canal of the left horn? 
 
Can't quite make out the measurements, was there an embryo in each horn?
