Borderline or low malignant potential epithelial cancers represent approximately 15-20% of all malignant epithelial neoplasms. They typically occur in the younger population with an average age of 35 to 40 years. Patients are often asymptomatic or present with a history of local symptoms including pelvic pressure or abdominal distention.
Borderline tumors are confined to the ovary in 65 to 90% of cases at the time of diagnosis. Accordingly, aggressive cytoreductive surgery is usually not necessary. However, it is very important to identify extra ovarian disease. These tumors do require formal staging at the time of surgery.
A total abdominal hysterectomy and bilateral salpingo-oophorectomy is typically performed for the patient who has completed childbearing. A unilateral salpingo-oophorectomy with staging is a reasonable approach in a younger patient who desires fertility.(84), (85)
