Details of responsibilities in current post

Routine responsibilities of a consultant in a busy O&G unit delivering just over 4,000 women per year.

My particular responsibilities are in the running of labour ward and in antenatal diagnosis, where I support the our biochemical screening programme for Down's syndrome and perform all routine amniocenteses. Currently I am collaborating with colleagues at the Institute of Epidemiology ("Leeds") in the GRIT study of management of preterm intrauterine growth retardation. I recently acted as supervisor to a postgraduate radiography student for her MSc research dissertation. I have recently produced a protocol for ultrasound scans within the department. I am currently developing a new labour ward protocol. This follows recent discussions arising from the RCOG's document on minimum standards in delivery suite practice.

I am currently involved in the following initiatives: developing universal antenatal screening for Hepatitis B infection with a protocol for management of Hepatitis B detected by screening; (now completed - audit planned); adaptation of current facilities to bereavement (complete) and high- dependency patient areas (in progress); planning facilities for "waterbirths"; "Bleep-free" teaching of junior staff; (now in place including joint registrar teaching between Luton and Bedford) "Hospital-at-Home" scheme for early discharge after gynaecological surgery; responding to "Changing Childbirth", developing other patterns of maternity care.

Special Experience

I was fortunate at Reading to have been able to observe and help to further develop a scheme for midwife or GP/midwife administered ante-, intra- and post-partum care. This has given me an insight into the attractions and pitfalls of some aspects of the recent reports of the parliamentary health committee and of Baroness Cumberledge. I am now involved locally in similar developments.

As senior registrar I was very involved in labour ward management and supervision of registrars and SHOs. I enjoyed a good working relationship with the consultant obstetric anaesthetist enabling me to develop an interest and expertise in the management of many cases of severe pre-eclampsia. Currently I have clinical responsibility for the day-to-day running of the delivery suite, and have two clinical sessions per week on delivery suite.

During my two years as a research fellow I worked evening and weekend sessions in clinics run by the Marie Stopes organisation dealing with family planning matters and counselling of women with unplanned pregnancy. During this work I carried out research which resulted in my being asked to take part in a number of TV and radio programmes and press coverage. Subsequently I have been involved in a number of media interviews both concerning my research and also in matters concerning my current employers.

I have experience of mid-trimester surgical termination of pregnancy. I have assisted in the training of a family planning doctor (CMO) to enable the Reading family planning clinic to establish arrangements for both medical (Mefipristone) & surgical termination of pregnancy and laparoscopic sterilisation. I continue to provide a service for mid- trimester surgical termination of pregnancy.

I was the medical member of a group developing a training programme for nurse first assistants in O & G theatres at Reading. It is hoped that this scheme may later be extended to other specialities and units. The first trainees began the programme on 1st November 1993.

My recent work concerning seat belts in pregnancy sparked off a high level of media interest and lead to numerous articles in magazines, national & local press and I was involved in several radio broadcasts.

I was approached by Family Doctor Publications (an off-shoot of BMJ Publishing) to write a book for the lay-reader on Women's Health Matters. The book is co-authored by my wife and is due to be published early in 1996.

Obstetric Risk Management Group

As a senior registrar I established a risk management group in Reading. In my current post I was able to establish the post of risk- management practitioner. An appointment to this post has now been made. Together with the appointee and our directorate business manager we have established effective risk management within our directorate. Our experience is now being used as a basis for developing risk management throughout the trust, a process in which I am very much involved, supporting the medical and nursing directors. I am a member of the Trust's Clinical Risk Management Group. Currently I am involved in organising a regional study day on risk management for senior clinicians and managers.

Antenatal Diagnosis

I have had a large experience of routine amniocenteses - currently performing about 200 per year, using both ultrasound-guided and -directed techniques. At Wexham Park Hospital (under Mr. Reginald's supervision) I learnt to perform "early" amniocentesis. Using the techniques learnt I was able to apply a neglected method of non-invasive gestational sac volume measurement, to demonstrate replenishment of amniotic fluid after amniocentesis. Our paper has since been published. At Reading I worked closely with the maternity ultrasound staff and took responsibility for management of most pregnancies complicated by congenital abnormalities diagnosed antenatally. I supervised the pregnancy termination and follow up of most of these women. In my current post I perform all amniocenteses, supervise the obstetric ultrasound unit and all aspects of antenatal diagnosis.

Bereavement Counselling

The department in Reading particularly supported bereaved parents and those who have had a pregnancy terminated for congenital abnormality. There was a "stillbirth group", consisting of hospital and community midwives, a nursery nurse, clerical staff, clergy and myself as medical representative. The group was instrumental in making wide-ranging changes and establishing protocols. Special ("homely") rooms were equipped for the care and counselling of bereaved couples. With the support of my consultant colleagues I established a pregnancy loss clinic for these couples. The clinic offered support and clinical advice regarding recurrence risk and genetic counselling. There was liaison with the regional genetics service. I am currently reviewing the throughput of this clinic, through which have passed more than two hundred women since its inception. I subsequently managed the "high risk" pregnancies among many of the couples previously seen in this clinic, generally with successful and satisfying outcomes. Shortly before leaving Reading I handed over responsibility for this clinic to a part-time colleague. I am currently extending my experience and practice in this area in Luton. I am currently applying for research funding to investigate incidence of morbid grief and pregnancy outcomes following bereavement, as well as the contribution of obstetric cholestasis to "unexplained stillbirth".

Management & Health Economics

I attended the management course organised by Oxford RHA for senior registrars, which I found very useful and have begun to try to apply to my work. I attended a one day course on "Business Planning in Obstetrics & Gynaecology" held at the Royal Society of Medicine. This course was designed for clinical directors and business managers in O & G.

At Reading I was involved in aspects of management within the department, particularly with regard to junior medical staff. I have assisted the clinical director in successfully bidding for two additional consultant posts from regional funds. I organised the changes in the medical staff rotas required to reduce junior doctors hours. I actively participated in divisional meetings and regional speciality sub-committee meetings, and deputised for my consultant colleagues at regional colposcopy meetings.

I believe that clinicians need to take an active role in management and priority setting and that an appreciation of management and financial skills as well as economic principles will allow us to do so. I decided that I needed to develop "business" and management expertise. I enrolled on a "distance learning" course run by Aberdeen University towards a certificate in Health Economics, and was awarded a scholarship. The course has given me a new insight and also given me experience of a new learning method which I pursued further via the Open University.

The next year I enrolled as a student at the Open University Business School to study for a Professional Certificate in Management (with a bias towards Health Service Management). I completed this course in October (1994), I was awarded the Cert.Mgmt.(Open) in Health Care. I hope through further study to obtain an MBA.

In my current post I very much support the clinical director, occasionally deputising for him and have responsibility for risk management and other activities in the directorate.

Medico-legal Work

I have long been interested in the legal process and in medico-legal issues. Soon after taking up my current post I attended a medico-legal conference. As a result of my interest at that meeting I have been approached by solicitors from a number of firms to act as an expert witness. I attended a seminar at Lincoln's Inn, where I won the prize for the most effective expert witness in a series of "mock trials". I also attended other symposia at the RCOG and elsewhere concerning medico-legal issues and risk management. I see this activity as intellectually stimulating. It has the potential to benefit patients, but also to highlight potential problems in my own practice and within my unit - benefiting from the mistakes of others. This will also be useful to me in the development of risk-management locally.

I am planning to attend the University of Glasgow's residential school for the Certificate in Medical Law later this year.

Administration

In most of my previous posts I have been responsible for organising the "on-call" rota. At Guy's I frequently deputised for my consultant and represented the department on an ad hoc cytology committee. Here I also organised clinic timetables and departmental meetings. At the Hammersmith I organised the weekly postgraduate sessions (with visiting speakers), weekly pathology meetings and an induction course for new SHO's. In Reading I organised our weekly postgraduate half day meetings (drug company presentations, gynae-pathology & -radiology meetings, audit, perinatal mortality and morbidity meetings, visiting speakers, journal club and case presentations).

Also in Reading, I represented the clinical directorate on a committee concerned with the redesign and standardisation of pathology request forms, possibly aimed towards a single unitary request form. I was a member of a working party working with a management consultant to assess staff numbers and skill mix in the labour ward at Royal Berks Hospital. The review addressed staff of all grades and in both midwifery and medical disciplines.

Currently I supervise the joint Luton/Bedford registrar teaching which involves liaison with colleagues in Bedford. I organise a twice yearly meeting of all Bedfordshire O&G consultants.

Medical Audit

Recent health service changes and intellectual interest in medical audit have given this field much greater importance ( and some financial support ). With the help of the district clinical tutor and audit assistants ( in East Berkshire ) I began to develop an interest in this aspect of medical care and have been involved in audit projects. At Wexham I was responsible for audit in the department.

In Reading I was able to continue this interest. I have been involved in audit of the following areas of care - discharge after Caesarean section, correct completion of discharge letters, administration of antenatal steroids, transfers of care in labour.

Particularly in the audit of steroid administration, changes in management have resulted and in collaboration with the district audit unit this activity was later re-audited, and a considerable improvement was demonstrated.

In Oxford I was asked to lecture to junior staff on "Audit in Obstetrics & Gynaecology".

Currently I sit on the Trust's audit committee and lead audit within the directorate, and negotiate "purchased audits" with Bedfordshire Health. I have been invited by the Director of R&D of the NHS Executive to attend a residential workshop on Clinical Efectiveness.

Teaching

In most of my previous posts I have been involved in formal and/or informal teaching medical students, junior medical or nursing staff. At Guy's I lectured regularly to medical students and postgraduates. At Wexham I lectured to student midwives, qualified nursing staff on ENB courses and to parents-to-be in the department's antenatal classes. In Reading I gave weekly tutorials to SHO's and regularly lectured to student midwives. In my most recent posts I have marked essays and coached candidates for the MRCOG and DRCOG examinations. I have taught on a London-based MRCOG part II course.

In connection with my research I have been invited to lecture to a number of postgraduate meetings. I was involved in a course run by the British Postgraduate Medical Federation on colposcopy for Genitourinary physicians.

In Reading I introduced an induction course for new SHO's, which is to continue. In Oxford we always had a group of students attached to our firm and I enjoyed formal and informal teaching of them.

I currently organising a programme of weekly half-day release teaching sessions for career trainees in O&G at both our own Hospital and also at Bedford Hospital. I am educational supervisor to two SHO's and one registrar.

Diabetic Antenatal Care

At Reading we enjoyed the support of the diabetic team. A weekly early morning clinic was run, staffed by Dr Hugh Simpson (diabetic physician), myself and one or two of the diabetic nurse specialists. The clinic had a large throughput of cases, increasing each year (about 50 new patients per year). Patients were seen weekly to monthly according to their needs. Changes were made to management protocols. Audit was on- going.

I was invited to lecture to GPs, nurses and dieticians on preconceptual and antenatal care in diabetes. I currently represent our directorate on the District Diabetic Forum.

Surgical Experience

I have had a large and wide experience of all aspects of gynaecological surgery. Together with one of my consultant colleagues I introduced the technique of sacrocolpopexy for vaginal vault prolapse. I was fortunate to have received tuition in the technique of endometrial resection by Mr E.M. Holt, who is a national authority on the technique.

Editorial Work

I have acted as a reviewer for papers submitted to the British Medical Journal, British Journal of Obstetrics & Gynaecology, Journal of Obstetrics & Gynaecology, British Journal of Family Planning, and Journal of Medical Screening, I have reviewed books for British Journal of Family Planning and British Journal of Sexual Medicine and a television programme for the British Medical Journal.

College Activities

I have regularly attended and taken an active part in RCOG meetings. After recent discussions concerning trainees' representation, I was asked to organise an inaugural Oxford regional meeting of trainees, district tutors, regional college adviser, other RCOG representatives and postgraduate dean. The meeting took place in November 1993 and as a result I became a member of the new regional and national trainees committees. I had to resign from both of these post on appointment as consultant, but have continued to support the regional trainees' committee.

Current Research Interests

1) Identification of patients most likely to benefit from a policy of selective antibiotic prophylaxis for Caesarean section. (Based at Wexham Park Hospital - part funded by a grant from Oxford RHA) (Collaborator - Miss T.Batool)

2) Assessment of the possible role for some antenatal care to be "self- administered" by pregnant women. (Collaborators - Miss H.Allott, Mr A.N.Boulos & Mr E.M.Holt at RBH - paper submitted)

3) Value of "routine" post mortem examination after second trimester fetal losses, and choice of method of termination and postmortem for second trimester diagnoses of congenital abnormalities.

4) Review of 200 cases seen in a special "bereavement" clinic after second and third trimester losses. To assess the value of various aspects of management and investigation. With particular reference to pregnancy outcomes, morbid grief and pregnancy spacing.

5) Growth restriction intervention trial (GRIT). A multi-centred, multi-national randomised trial of management of intra-uterine growth retardation.

Previous Research

As shown by my list of publications I have carried out research into human papilloma virus, cervical intraepithelial neoplasia, sexually transmitted diseases, colposcopy, cervical cytology, the etiology of cervical cancer, contraception, "early" amniocentesis and road safety aspects of pregnancy. Much of this work has been unsupervised and/or carried in a district general hospital setting.

Research Grants

I held a grant from Oxford Regional Health Authority's "Locally Organised Research" Scheme, for work concerning the factors which predispose to septic complications after Caesarean section.

Early Pregnancy Assessment Clinic

In Reading there was a "same-day" appointment clinic for complications of early pregnancy. The clinic was manned by one of the gynaecology SHO's in turn. The clinic was under the supervision of the senior registrar. Vaginal ultrasound scanning and serum BetaHCG assay are available in the clinic. A similar system operates in Luton.

"Minor Operations"

At Reading I set up a weekly "minor ops" clinic, which received direct referrals for procedures not requiring general anaesthesia (excision of cysts, skin-tags, polyps & cautery). The clinic also received referrals for investigation of post-menopausal bleeding. Guidelines for management using vaginal ultrasound and 'Pipelle' biopsy were established. I have personally performed more than 600 such procedures.

Presentations to Learned Societies

I have made a number of oral and poster presentations arising from my research work at meetings of the British Society for Colposcopy & Cervical Pathologists and the Medical Society for the Study of Venereal Diseases.

Colposcopy

I have a considerable experience of the use of colposcopy, both in females and males. Allied to this I am familiar with the use of laser (excision and ablation), cold coagulation, cryotherapy and large loop excision in the treatment of intraepithelial neoplasia.

1/02/94 - 30/06/94 John Radcliffe & Churchill Hospitals-Senior Registrar in Obstetrics, Gynaecology & Gynaecological Oncology to Mr F.M.L.Charnock

This post completed my senior registrar rotation. It is a busy unit with between 6,500 and 7,000 deliveries per year. Though relatively brief it nevertheless gave me a chance to experience a different administrative and clinical approach, in a large busy department, as well as exposing me to a teaching hospital environment. Usefully I was involved in a number of peripheral clinics. I worked with Mark Charnock who has a special interest in gynaecological oncology. With him I enjoyed involvement in joint oncology clinics and some further exposure to radical surgery, as well as his empathetic and holistic approach.

As senior registrar in a single "firm" I enjoyed having below me a full registrar & SHO team, preparing me for a similar position as consultant. During my sojourn in Oxford I was involved in undergraduate and postgraduate teaching, gave lectures, organised perinatal meetings, made surgical grand round presentations and presented a paper at the regional O & G scientific forum.

1/02/92 - 31/1/94 Royal Berkshire Hospital-Senior Registrar in Obstetrics & Gynaecology to Messrs. Holt,Greenhalf,Williams,Crystal & Selinger

This was a busy unit delivering 5,500 maternity cases a year. There was also a very heavy gynaecological workload. The unit has made developments in the delivery of midwife and GP oriented maternity care enabling obstetricians to focus on high risk cases. In gynaecology minimally invasive techniques were in use and the majority of operative cases were day-cases, making use of a dedicated gynaecological day case unit.

I was able to develop further special interests in neonatal loss and genetic counselling. Administratively my responsibility was to assist the consultants in the continued smooth running of the department. I attended divisional meetings and deputised for my consultants at regional meetings. I successfully assisted the clinical director and business manager in preparing a bid for funding for a new consultant appointment ( obstetrics only ), and a further successful bid for a part-time appointment ( now pending ). These steps, together with other changes allowed me to devise ways to cut junior doctors hours in line with recent national agreements.

Clinically I had regular commitments in an antenatal clinic, gynaecology clinic, labour ward and theatre sessions. Despite the heavy clinical and administrative responsibilities I was able to continue some of my research interests.

Career Aim

I am pleased to have achieved one of my career aims of a consultant post in Obstetrics and Gynaecology in a busy district general hospital, maintaining wide ranging interests and a commitment to labour ward practice and management of pregnancy loss. I wish to extend myself in this role by providing a first class clinical service, both within the NHS and outside it.

I would also aim eventually to have a substantive management appointment. I believe that supervision and a more structured training of junior staff are essential and would be keen to be involved in such a role.

Outside Interests

Wine, collecting antique maps & corkscrews, DIY, rambling and cycling.

Personal

I am married. Until our move to Luton my wife was a partner in general practice in High Wycombe. She has particular interests in terminal care and family planning. She sat on the Joint Advisory Group on terminal care of the local health authority and county council, was a member of the medical advisory committee of our local hospice and had a weekly session in a family planning clinic at our cottage hospital. She hopes shortly to work part-time after a brief career break. Our first child - David - was born in September 1991, William was born in November 1995.
